From providing the convenience of setting up your own appointment time to ensuring that all of your required documentation is complete, we are driven to make your DOT physical experience as pleasant and problem free as reasonably possible. We are currently accepting contracts for DOT physicals to be delivered at your place of business only.

Individual DOT physicals by appointment only.

All physicals and submissions conducted and made by a fully licensed and registered Certified Medical Examiner and in compliance with the Federal Motor Carrier Safety Administration guidelines.

What to bring

A valid TN drivers license.

N

A list of your medical conditions and medications including dose and frequency.

If you have insulin controlled diabetes, you must bring, Blood glucose and A1C logs (doctor's records, lab report, etc.), glucometer any other relevant documentation

Please bring any prescription eye glasses, contacts and working hearing aids.

If you use a C-pap machine, bring your latest Sleep Specialist report, your C-pap machine correct usage documentation (bring at least the last 90 days of records). Drivers who have or have had heart, lung or brain conditions requiring specialist care, (e.g. stroke, seizure, emphysema, COPD, stents, MI, etc.) may be required to get clearance documents from their specialist(s).

A full bladder if an Urine Drug Screen is required.

Please arrive dressed appropriately for a complete non-invasive physical examination.
You can expect to spend 45-60 minutes at the clinic to complete your examination.

Each DOT physical is unique inasmuch as each person is an individual who may or may not have different physical limitations or concerns. Completion of the DOT physical exam requirements does not guarantee successful passage. Successful passage is fully dependent on the results of your physical as they relate to your ability to safely operate vehicles requiring a CDL in the State of TN.

What to expect during and after the physical

When going to a DOT physical, there are things you must bring with you.

On the day of your exam, please bring,
